Visits

We are so happy that you have chosen Sugarloaf Pediatrics, PC to provide your son/daughter’s medical care.  Our website contains all of the vital information you will need to manage your child’s care within our practice.  Please refer to our Appointment Policy. For visits to our office, please plan to arrive 15 minutes prior to the beginning of your appointment to verify demographic information, make your co-payment, and fill out any necessary forms. Please print out necessary forms for each visit and fill them out prior to your arrival.  Please make sure to complete CHADIS surveys prior to appointment if required.  This will ensure optimal times spent with the doctor. If you do not have access to a printer, please arrive early to fill out necessary forms.

For your child’s first visit to the office, please fill out and bring:

For subsequent visits to the office, please fill out and bring the following:

  • Immunization Record Book
  • Update Demographics Form if anything has changed since your last visit
  • For Well Care Visits, please go to: www.CHADIS.com and complete all of the assigned questionnaires for your child

When calling to schedule an appointment, it is important to state the type of appointment you wish to make.  Below is a description of appointment types:

  • Well Care Visit
    These appointments are scheduled to last for 30 minutes.  Dr. O’Brien will first meet with the parent and child to discuss history and concerns.  Then she will perform the physical examination.  As children get older, they reach a time when they prefer that their parent is not in the room during their exam.  Up until 13, this decision is left up to the child.  However at 13, Dr. O’Brien prefers that the parent step out of the room for the exam and then rejoin their child after the exam.  This time allows your child to discuss any concerns or questions that they may not feel comfortable discussing in front of their parent.  If your child does not feel comfortable with this, their parent is more than welcome to stay during the exam.  If there is not a parent in the room during the exam, Dr. O’Brien will have a nurse assist her if needed. 

    Most insurance companies will only allow 1 well care visit (or physical exam) per calendar year.  In addition, some insurance plans require a co-payment for well care visits while others do not.  It is the parent/guardian’s responsibility to check with their insurance company prior to the well care appointment to assure that the visit will be covered and to determine if a co-payment is required.
  • Sick or Follow-Up Visits
    Sick or follow-up visits are typically schedule for 15 minutes.  Examples of types of visits scheduled in this category are:                                                                                                             
    • Acute illness visit
    • Recheck after illness (i.e. ear infection, pneumonia, asthma flare)
    • Oral Contraceptive follow-up
  • Consultations
    These appointments are scheduled for 30 minutes.  Examples of types of visits that would be considered a consultation are:
    • Behavior Issues
    • Toileting Issues
    • Learning Issues
    • Emotional Issues
    • Abdominal Pain > 2 weeks duration
    • Headaches > 2 weeks duration
    • Eating Disorder
    • Irregular menses
    • Chronic medication checks
    • Chronic asthma management
    • Gynecology issues (new birth control start, pap smear, STD testing)
    • Pregnancy

It is important that you bring your child to these consultation appointments.  Dr. O’Brien is more than willing to meet with the parents alone if needed during the visit. However, it is equally important to hear the child’s viewpoint and perform a physical exam if needed.
